NOTICE TO VOTERS:

PHOTO ID IS REQUIRED
THE ID MUST INCLUDE THE FOLLOWING INFORMATION:

1. PHOTOGRAPH OF VOTER

2. INCLUDE NAME WHICH MUST CONFORM WITH YOUR NAME ON YOUR VOTER REGISTRATION RECORD

3. HAVE AN EXPIRATION DATE AFTER NOVEMBER 2, 2010

4. BE ISSUED BY THE U.S. GOVERNMENT OR STATE OF INDIANA, IF YOU DO NOT HAVE A PHOTO ID WHEN YOU APPPEAR TO VOTE, YOU WILL CAST A "PROVISIONAL BALLOT" WHICH PERMITS YOU TO VOTE. YOU MUST PRESENT A PHOTO ID TO YOUR COUNTY ELECTION BOARD NO LATER THAN 10 DAYS AFTER ELECTION DAY FOR YOUR "PROVISIONAL BALLOT" TO BE COUNTED.
